Insurance Information

For your convenience, Physical Therapy Advantage will file all claims directly with the patient’s insurance carrier. Once the insurance has processed the claims, the patient will be billed and required to pay for all co-insurance, and/or any amounts that are not paid by the insurance carrier. Co-pays are due at the time the service is provided. 

Physical Therapy Advantage will obtain a quote of benefits from the insurance carrier and report this information to the patient upon the first or second treatment session. Please remember a quote of benefits is not a guarantee of payment or coverage. It is the patient's responsibility to contact his/her insurance carrier with any discrepancies in coverage.

We are a participating provider with the following insurance carriers:

  • Blue Cross/Blue Shield (PPO and POS);
  • Medicare;
  • Worker’s Compensation;
  • Tricare (Military);
  • HealthCare's Finest Network (HFN).

If you do not see your medical insurance listed above, please call us at (630) 892-8003. We are in the process of adding new contracts as we grow and we may not have updated this list to match our current network status.
